For decades, Iowa Health and Human Services (HHS) has used the Iowa Youth Survey (IYS) to better understand the attitudes and beliefs, experiences and behaviors of Iowa's adolescents. When enough 6th, 8th, and 11th grade students participate from each school district, HHS can provide district-specific results that reflect your students’ perspectives and experiences. Your district, community, and state can use the information to provide or maintain data-driven programs that keep your youth safe, healthy, and hopeful about their future.
School administrators, please complete the following form and indicate whether or not your district will participate in the Fall 2025 Iowa Youth Survey.
The 2025 Iowa Youth Survey will be administered between September 15 – November 10, 2025. Please register by October 17, 2025. All participating districts must collect active parental consent prior to survey administration. For more information on the Iowa Youth Survey, visit www.hhs.iowa.gov/iowa-youth-survey.
The Iowa Youth Survey is a partnership between Iowa Health and Human Services and the Iowa Department of Education. Iowa State University is a contractor of Iowa HHS and responsible for providing technical assistance, distribution, and communication for the 2025 IYS.
