Please take a moment to schedule fall Iowa Youth Survey administration for your district or school, which will take place over approximately 30 minutes between September 15 – November 10, 2025. Your superintendent already approved your district's participation in the fall 2025 Iowa Youth Survey.
The information that you provide on this form will help us to support your school leading up to survey administration on the date/time of your choosing. If this information changes in the future, just reach out with updates.

The Iowa Youth Survey is a partnership between Iowa Health and Human Services and the Iowa Department of Education. Iowa State University is a contractor of Iowa HHS and responsible for providing technical assistance, distribution, and communication for the 2025 IYS.
